What is cortisol?

Cortisol is a steroid hormone made by the adrenal glands, two small organs that sit on top of your kidneys. It is produced in the outer layer of these glands, the adrenal cortex, and released into the bloodstream in a rhythmic pattern across the day. Cortisol belongs to a group of hormones called glucocorticoids, which help regulate metabolism, blood pressure and the immune system.

Your body's stress responder

Cortisol is often described as the 'stress hormone' because it rises in response to physical or emotional demands. It does far more than manage stress, though. It helps control blood sugar, supports blood pressure, moderates inflammation, and influences how the body uses fats, proteins and carbohydrates for energy.

The daily rhythm that keeps you running

Cortisol follows a natural circadian cycle. It peaks in the early morning to help you wake and become alert, then gradually declines through the day to reach its lowest point around midnight. Because of this rhythm, the time of day a sample is taken is central to interpreting the result. A value that is normal at 8am could be unexpected at 10pm, and vice versa.

Why cortisol matters

Cortisol sits at the intersection of your brain, metabolism, immune system and cardiovascular health. It is governed by a feedback loop known as the hypothalamic-pituitary-adrenal (HPA) axis, which fine-tunes how much is released. When this system is balanced, cortisol helps you meet the demands of the day and recover afterwards. Persistent imbalance, in either direction, can affect energy, mood, weight, blood pressure and bone strength over time.

What your results can mean

Lower than expected

A low result usually reflects reduced adrenal output or altered signalling along the HPA axis. This can relate to the adrenal glands themselves, to the pituitary gland, or to the effects of long-term corticosteroid medication. Lower cortisol may be associated with fatigue, low blood pressure, poor tolerance of stress or illness, and difficulty maintaining blood sugar when fasting. A clinician considers these patterns alongside other tests before drawing any conclusion.

Within the expected range

A result within the reference range for the time of the draw suggests the adrenal glands are responding appropriately to the body's demands at that moment. Because cortisol is highest in the morning, a healthy morning value typically sits in the mid to upper part of the range, reflecting a well-functioning stress response and metabolic regulation.

Higher than expected

A high result can reflect ordinary physiological stress, acute illness, pregnancy, or certain medications, as well as less common processes affecting the adrenal or pituitary glands. Because so many everyday factors raise cortisol, a single elevated value is rarely meaningful on its own and is usually followed up with timed or dynamic testing directed by a clinician.

What influences the result

Cortisol is unusually sensitive to context. The main influences on interpretation are the timing of the blood draw, recent stress, illness or intense exercise, sleep and shift-work patterns, pregnancy, and medications including the oral contraceptive pill and corticosteroids. Because of this, single random values are far less informative than samples taken at a defined time, or the dynamic tests a clinician may arrange.

Reference ranges in Australia

Australian laboratories report cortisol in SI units, nanomoles per litre (nmol/L). As a general guide for a morning serum sample, many laboratories use a reference interval in the region of 130 to 550 nmol/L at around 8 to 9am, with lower values expected later in the day. These figures are indicative only. Reference ranges are specific to each laboratory, assay and collection time, so always read your result against the range printed on your own report.
